WebSocket subscriptions at scale

Indexers and trading bots live on WebSocket subscriptions, and nothing hurts more than a silent disconnect.

The Nadcab gateway terminates client sockets itself and proxies frames to the upstream pool, metering each message by method so subscriptions are billed fairly.

When an upstream connection drops, the client is told immediately rather than left hanging, so reconnect logic stays simple and predictable.
